How to Register with

EIU Student Disability Services

To begin the registration process with our office:

1. SUBMIT the following to SDS:

   1. Documentation (include ONE):

      1. An IEP or 504 plan that is less than 10 years old

      2. OR documentation from the appropriate professional. To obtain forms or documentation guidelines follow the link below for your specific disability.

         • Attention Deficit Disorders
         • Autism/Asperger’s Syndrome
         • Blind/Low Vision
         • Deaf/Hard of Hearing
         • Medical Disabilities
         • Physical Disabilities
         • Psychiatric Disorders
         • Specific Learning Disability
