Guidelines for Documentation

Psychiatric Disorders

Students who have been diagnosed as having a psychiatric disorder may be eligible to receive services through the Office of Student Disability Services if the disability significantly limits major life activities. Examples of psychiatric disorders are major depression, anxiety disorders, personality disorders, and adjustment disorders.

To begin the registration process with our office, please submit the following to SDS:

   1. Documentation (include ONE):

      1. An IEP or 504 plan that is less than 10 years old

      2. OR Psychological Documenation Form completed by a qualified mental health professional   Word     PDF
