Purpose and Criteria
The Dale and Deb Wolf Outstanding Service Award has been established to honor and celebrate an annual recipient committed to applied learning through service to the department, college, community, and/or profession.
The award will be presented to a faculty member in CHHS who has enthusiastically contributed superior service while consistently promoting personal, community, and global health.
Each nomination shall include evidence of the nominee’s dedicated service in at least two of the following service domains during the academic year:

Eligibility
The nominee for the Dale and Deb Wolf Outstanding Service Award must be a faculty member (tenured, tenure-track, or annually contracted) in the College of Health and Human Services who has completed at least three consecutive academic years of full-time employment at Eastern Illinois University and who has not received the award within the past five years.
Nomination Process

Any College of Health and Human Services faculty member or department chair may nominate one eligible colleague for the award. Self-nominations will also be accepted.

Selection and Awarding Process
Incomplete or late applications will not be accepted or considered for the award.
An initial ad-hoc selection committee consisting of three-to-five full-time faculty members (preferably with no more than one faculty member per CHHS academic unit) will be populated by the Dean’s Office to review nominations. The selection committee will recommend an award recipient to the Dean of the College of Health and Human Services, who will inform the recipient of the selection decision.
Each award recipient will be announced college-wide by the CHHS Dean and honored at an annual CHHS event or celebration.
Mr. Dale Wolf and Dr. Deb Wolf (donors) will be invited to present the award to the annual recipient.
Recipients of the award will serve on the ad-hoc selection committee during their five-year period of ineligibility.

Dale and Deb Wolf dedicated the majority of their professional careers to the success of Eastern Illinois University: Dale as Director of Admissions for over 20 years and Deb as a tenured faculty member in the Department of Physical Education (now KSR). They share a continuing passion for service to university, community, and profession. Dale's extensive service initiatives have included serving as president of the Panther Club (EIU Athletics), IACRAO (Illinois Association of Collegiate Records and Admissions offices), the Charleston Chamber of Commerce, the EIU Annuitants Association, and CASA of East Central Illinois. Dale has been awarded the Charleston Chamber of Commerce Citizen of the Year (2010), Glen Hessler Award from EIU Athletics (2012) and the Jefferson Award (National Service Award). Deb's extensive service contributions have included serving as president and vice-president of the Illinois Association for Health, Physical Education, Recreation, and Dance (IAHPERD), Charleston Swim Club Advisory Board Member, American Red Cross Board Member, Genesis Animal Rescue Board Member, EIU Faculty Senate and EIU Library Advisory Board. Deb has been the recipient of multiple EIU Achievement and Contribution Awards in the category of 'Service' and has been the recipient of both the IAHPERD Service Award and the IAHPERD Honor Award.
